Attendance Forms

If your child is ill and cannot attend school, please call (0208 444 8488) or email (office@muswell-hill.haringey.sch.uk) the School Office by 9.00am on the day of illness.

Term time absences for more than one day - Requests for term time absence should be made using this Absence Request Form.    

Absences for one day/part of a day - If your child needs to miss all or part of a day for medical appointments, dental appointments etc that can't be made outside of school time, please complete an Application for an Absence of Leave Form. The completed form should be returned to the School Office, marked for the attention of Elaine Mason.

Daily absence reporting online is not yet available but we are working on it and hope to have it up and running soon.
